- Poor asthma control or persistent exacerbations despite regular daily moderate inhaled corticosteroids
OR
- Children with an unclear asthma diagnosis despite guideline-based workup

Complete the referral form
Complete the referral form online or print it out and complete it by hand. The form can be saved for your future use.

Include tests
Include any previous lung function tests, chest x-ray reports or allergy consultations with the referral.

Wait for an appointment
Once the referral is received, the clinic will call the patient to book the appointment (current wait time for non-urgent appointments is 3-4 months).
While awaiting an appointment, please refer to the Provincial Pediatric Asthma Guidelines for suggestions on asthma management.
