Please contact your primary health care provider or an optometrist to discuss your eye health concerns and request a referral to our clinic.

1Write a referral letter

Physicians and optometrists, please fax your letter outlining your reason for referring the patient.

For Orthoptics, download the referral form. Please note: the Orthoptic Department can only accept referrals from an Ophthalmologist.

2Fax the letter to one of our specialists

Fax the letter or form to one of our specialists (see the Ophthalmology Contact List). Our specialists include:

3For urgent referrals

For urgent referrals, call Paging at 604-875-2345 and ask for Ophthalmology.

4Follow-up with your patient

The clinic will contact the patient directly to make the appointment. Due to the high number of referrals, we cannot always meet our target time frame with all patients. Our non-urgent waitlist is generally 18 months.
