Referral overview
Patients need a referral from a health-care provider to access this service.
Referral steps

Assessment
You can refer children and youth to us if they are suffering with:
- Complex/chronic pain with a known or identified cause or diagnosis (e.g. oncology, gastroenterology, rheumatology, neurology, post-infectious/trauma-related)
- Complex/chronic pain without a known cause or diagnosis that includes but is not limited to unexplained pain, complex psychopharmacology, or diagnostic uncertainty
- Complex/chronic pain in the context of a developmental disability (cerebral palsy, brain injury, autism spectrum disorder)
- Pain in the context of psychiatric illness/concurrent disorder (somatization, substance use)

Complete a form
We need a completed referral form (PDF) to process a referral for both inpatients and outpatients. We will process community referrals and in-hospital consults when you have provided all information. Please include all consult notes.

Post referral
We will contact patients and ask them to complete an online questionnaire. Please note that they must complete this before the appointment.
Once the questionnaire is complete, we can book a clinic appointment.

We provide services from Monday to Friday, 8 a.m. to 4 p.m.
If you are a patient, parent or caregiver, please contact the clinic so they can transfer you to the correct health-care provider for support.
If you are a provider, please contact the nurse clinicians or clinic.
