Early Motor Screening Referral
An occupational therapist or physiotherapist conducts a general movements assessment (GMA). General movements are patterns we can observe during a specific time in a baby’s development. They provide information on how the baby’s central nervous system is developing and predict later motor conditions.

Complete a form
Please complete the referral form (PDF) in full and ensure all patient information is current. Include consults, reports and/or chart notes. See the referral form for a complete list of requirements.
We will not process incomplete referrals.

Fax completed referrals to: 604-453-8320.

Post referral
If you have not already done so, let the parent or caregiver of the referred child know that we will contact them with an appointment time.
