Family Immunization Referral
The Family Immunization Clinic provides all publicly funded immunizations, including COVID-19 and seasonal influenza vaccines, at no cost to patients and their family members visiting the hospital.
Referral overview
Patients and family members are welcome to self-refer to the clinic by calling to book an appointment or by walking in. We have limited walk-in appointments available. We offer them on a first-come, first-served basis.
Consultations with doctors need a referral from a doctor, nurse practitioner or public health unit. Requests for inpatient immunizations for patients staying overnight at BC Children's Hospital also require a completed referral form.
Providers can write their own referral and fax it to the clinic. Please include all relevant details to ensure appropriate triage and decision-making from clinic staff.

Submit the referral form
Fax completed doctor and inpatient referrals to 604-875-2311 or email them to familyimmunizationclinic@cw.bc.ca. Please call the clinic if you are requesting a same-day appointment, as we may not be able to accommodate it.
For consultations or appointments with an immunization nurse, call 604-875-3000 or email familyimmunizationclinic@cw.bc.ca. We have limited walk-in appointments available on a first-come, first-served basis.
You do not need to take any further action after submitting a referral. The clinic will reach out directly to book an appointment.
