General Surgery Referral
We play an active role in treating birth defects, traumatic injuries, and vascular access. We also provide nursing support for gastrostomy tube insertion and care, stomas, bowel management and complex soft tissue wounds.
Referral overview
We accept referrals for children and youth up to the age of 17 who have surgical problems. We accept referrals from health-care providers who have evaluated a child or youth.

Write a referral
We do not require a specific referral form.
Referral packages should include:
- Pertinent patient history
- Past medical history
- Physical exam findings
- Any diagnostic imaging or laboratory tests
Please make sure that the parent or caregiver contact information is accurate. Advise us if the patient you are referring to our clinic will be leaving the country for an extended period in the near future.

Post referral
If there is a change to the condition of the patient you have referred for care, please fax your notes to the clinic so our doctors have the most up-to-date information. These changes can include but are not limited to:
- Changes to lesion size
- Hernia becoming visible more frequently or less easily reduced
- New imaging results that provide more up-to-date information

Appointment with a provider
Our clinic will contact the patient’s parent or caregiver to schedule an appointment.
