Referring providers do not need to specify or select the most appropriate clinic for a patient in their referral. BC Children's Hospital providers will do this as they review each referral.

Referral overview
Patients need a referral from a doctor or nurse practitioner to access this service.
We serve children under 18 years of age who live in BC or Yukon.
Referral steps

Write a referral request
You can print the referral form (PDF) or complete a fillable form (PDF) online. Please include supporting reports with the referral request, including:
- Blood test results from the last six months
- Urine tests and urinalysis from the last six months
- All cultures and sensitivities
- All medical imaging reports pertinent to the referral, such as kidney ultrasound
- Previous kidney biopsies
- Previous genetic testing results

Fax the referral
Fax the completed referral to BC Children’s Hospital’s Renal Program at 604-875-3649.
You can fax us at any time, seven days a week.
We will return any referrals that are missing important supporting reports; we will not review them until we receive the requested reports.

Referral triage
We process referrals from Monday to Friday, except for statutory holidays.
A nephrologist will review the referral information to determine how soon we should see the patient. If a patient’s condition changes, please notify us by fax and we will re-assess their care needs.
The nephrologist typically triages referrals within 7 to 10 business days.

Appointment booking
A member of our clerical team will book the appointment based on the nephrologist’s recommendation. We will contact your patient’s parent or caregiver with the appointment information. We will fax an appointment confirmation to referring physicians or nurse practitioners, if requested.
We may also ask you to order some additional tests before we see the referred patient in one of our clinics.
