Neurology Referral
The Neurological Care Centre provides state-of-the-art care for children and youth with disorders of the nervous system.
Patients need a direct referral from a doctor or nurse practitioner to access this service. We accept new referrals for children up to the age of 16.
Referral steps

Complete a form
Complete a referral form (PDF), completing all sections and including any relevant documentation. Fax it to the clinic at 604-875-2285.
For neurosurgery referrals, please fax the relevant history and any pertinent diagnostics to 604-875-3109.

Post referral
If there is a change in the referred patient's condition, please fax your notes to the clinic so they have the most up-to-date information.

Appointment with a provider
Our clinic will contact the patient's family to schedule an appointment date and time.
