Patients need a referral from a health-care provider or an optometrist to access services from the Ophthalmology Clinic.

Submit a referral
Ophthalmologist referral: fax a referral letter to 604-875-3561.
Orthoptic services: complete the orthoptic referral form (PDF) and fax it to 604-875-3561.
Electrophysiology: complete the visual electrophysiology requisition (PDF) and fax to 604-602-8651.

Post referral
After submitting a referral, no further action is required. The clinic will contact the patient and their family or caregiver directly to set up an appointment.
