Pediatric IV Outpatient Therapy (PIVOT) Referral
The PIVOT Clinic supports children and youth who meet the following criteria:
- Have central venous access (PICC, port, or tunnelled CVC)
- Require five or more days of IV antibiotics
- Otherwise medically stable and appropriate for discharge
- Have parents or caregivers interested in providing this care at home
- Have appropriate community resources to support them
What we do
We work with patients, community teams, and other teams across BC Children’s Hospital. Together, we treat the patient's infection and support home IV antibiotic treatment.
We order tests that will occur at BC Children’s Hospital. We also ensure that other BC Children’s Hospital teams follow up on test results as required. We make sure this information is shared with families and community teams.
In addition, we offer the following services:
- Home IV education: teach parents and caregivers how to manage a central line, heparin locking, flushing, cap changes, dressing changes, and training on various infusion devices
- Coordinate care: coordinate home IV medications, devices and supplies with the provincial home IV pharmacy, Calea. We also work with community medical centres closer to the patient's home. This helps ensure continuity of care
- Provide virtual health appointments as needed, for families to help troubleshoot pump or line issues
- Host weekly virtual health appointments with the PIVOT team. These sessions provide support throughout the program and address any concerns
- Develop clinical resources to help identify options for a plan of care to get the patient home or close to home (options for different types of devices, central lines, hybrid care models with community partners, etc.)

Send the referral
Fax the referral form to 236-429-3630.

Referral review
The PIVOT team will acknowledge receipt of the referral within one business day. Our team will complete the referral as soon as possible (generally within a few days but it may take longer if complex).

Follow-up
The PIVOT team will review all consults received before noon from Monday to Wednesday within one business day.
