Respiratory Medicine Referral
We provide care for conditions such as asthma, cystic fibrosis and primary ciliary dyskinesia. We also support children and youth who have sleep disordered breathing, tracheostomies and those who need home ventilation support.
Referral overview
Patients need a referral from a doctor or nurse practitioner to access respiratory medicine care at BC Children’s Hospital.
Providers: please submit one individual referral per patient for respiratory medicine specialist attention. Do not refer a patient to multiple clinics or services.
The respiratory medicine team accepts referrals for the following services:
- Pulmonary Function Laboratory
- Respiratory Medicine Consult Clinic
- Asthma Clinic
- Sleep Clinic
- Home Tracheostomy and Ventilation Clinic

Complete the referral form or draft a referral letter
For the Asthma Clinic, Pulmonary Function Laboratory and Sleep Clinic, please complete the relevant referral form: Asthma Clinic Referral Form (PDF), Request for Pulmonary Function Tests (PDF) or Requisition for Sleep Medicine Evaluation (PDF).
The Respiratory Medicine Consult Clinic and other chronic respiratory disease management clinics do not have a templated referral form. Please draft a letter providing an overview of your patient’s needs and a clear reason for the consultation request. Consider including:
- Pertinent medical history
- Any relevant documentation, including medical imaging, pulmonary function tests, and other reports
Please note: the Respiratory Medicine Consult Clinic prioritizes patients with severe, complex or rare breathing disorders based on the information provided in the referral letter. We may reject referrals if patients have less severe illness. The team may recommend additional testing or alternate referral pathways.

Post referral
For outpatient referrals, clinic staff will let the referring provider’s office know once we have received the referral, and again when we have made a decision.

Appointment with a provider
Families and providers do not need to take further action after a referring provider submits a referral form or letter. We will contact referring providers after triaging the referral. Clinic staff will contact patients and families directly to book an appointment date and date.
