We serve Grandview/Woodlands, Strathcona and the Downtown East Side.
Referral overview
Please use the same referral form for all services.
Anyone can refer to the primary care clinic: health-care professionals, community agencies and schools. Patients or parents/caregivers can also self-refer.
Our pediatric specialty services need a referral from a doctor or nurse practitioner.
There is one form for initial intake. We will request any additional materials or information after the initial triage.
If referring for intake post-discharge or from another health agency, please include any relevant hospital or medical records.

Submit the referral form
Fax the completed referral to 604-875-3958.

Review of referral form
The clinical team reviews and triages referrals frequently. Please update us with any changes in the patient’s condition and with changes of address, contact information and/or what school the child attends.
The family or referee need not take any action after referral. We will reach out to families/referees for further information, to book appointments or to redirect them as needed.
