We look at how these issues affect their comfort and how they participate in activities. We then discuss treatment options with families, including physical therapy, medication or surgery recommendations.
Referral overview
Patients need a referral from a doctor to use this service.
Referral steps

Complete a form
Please complete the referral form (PDF) and ensure all patient information is current. Include consults, reports, and chart notes (see referral form for requirements).
We will not process incomplete referrals.

Send the referral
Fax the completed referral form to the number listed on the form.

Post referral
Once received, we will review the referral to assess whether it meets our criteria and will then provide a receipt of the referral letter to the family.

Appointment with a provider
Our booking clerk will reach out to book an appointment for our next available time.
We ask parents and families to please let us know if there are any changes in a child’s condition before we book the appointment, such as surgery, illness or an increase or decrease in symptoms.
