Cerebral Palsy Early Diagnosis Referral
We provide assessments and examinations for early diagnosis of cerebral palsy.
Referral overview
To access our services, patients need a referral from a doctor.
The patient must meet all three of the intake criteria below:
- Younger than one year of age and
- Has an abnormal General Movement Assessment (GMA) result, and
- Demonstrates at least one of the risk factors in table 1 or table 2 (see referral form below)

Complete a form
Please complete the referral form (PDF) in full and ensure that all patient information is current. Include consults, reports, and/or chart notes (see referral form for requirements).
We will not process incomplete referrals.

Post referral
If you have not already done so, let your patient know that we will contact them with an appointment time.
