Complex Care Referral
The clinic treats children with:
- Genetic syndromes
- Neurologic impairments
- Multiple subspecialists involved in their care each year
- Frequent hospitalizations
- High-intensity care needs
Please note: we do not provide care for patients with cancer, transplants, or psychiatric and behavioural challenges.

Doctors and nurse practitioners can refer patients to this clinic.
Referral steps

Complete an assessment
Patients must meet the following criteria to be accepted to our clinic:
- Three or more sub-specialists actively following the patient. Note: “Actively” means outpatient visits at least twice a year
- A history of frequent visits and/or hospitalizations at BC Children’s Hospital
- High-intensity care and/or dependence on technology
- Under 16 years of age
- Not already primarily followed by a multidisciplinary clinic at BC Children’s Hospital (e.g. neuromuscular, spina bifida, oncology)
For patient referrals that do not fulfill all criteria, please contact us for further discussion.

Complete a form
Please complete the complex care referral form (PDF). Attach any relevant reports to the referral.

Email the referral and relevant documents to: BCCHComplexCare@cw.bc.ca, or fax to: 604-642-8828.

Post referral
We will prioritize and book accepted referrals based on patient need and clinic availability. The clinic will reach out to the patient's parent or caregiver to book an appointment.
