Positioning and Mobility Referral
Our services include:
- Assessments for children and youth from birth to 19 years of age
- Providing complex, customized equipment
- Working with families, children and youth, and community professionals to plan the best solution for patients
- Providing services either at Sunny Hill Health Centre or in a child's home community
Referral overview
We accept referrals from doctors, physical therapists and occupational therapists.
Providers within BC Children’s Hospital may initiate a referral internally through CST Cerner.
The primary contact is the child or youth's community physical therapist (PT) or occupational therapist (OT) from school or early intervention, who is responsible for providing specialized equipment. If the child requires custom-made seating or their equipment needs are more complex than the primary therapist’s capacity, our team will work with the primary therapist and family.
Please connect with the child's community OT or PT before making the referral, to confirm they need support from our team.

Complete a form
Complete the referral form (PDF) in full. Ensure all patient information is current. Include consults, reports, and/or chart notes (see referral form for requirements). Please indicate what positioning and mobility needs you have identified for the child or youth.
We will not process incomplete referrals.

Send the referral
Fax completed referrals to: 778-504-9768.

Post referral
Once we have received a referral, we triage patients based on urgency of equipment needs. We either assign them to a therapist, place them on the waitlist, or redirect them to a more appropriate service.

Appointment with a provider
Once we have processed and triaged the referral, a therapist will contact the patient and their family/caregiver directly for intake and to plan the initial appointment.
