Children or youth need a referral from a doctor or nurse practitioner to use this service.

Assessment
The Visual Impairment Clinic does not typically accept referrals for children or youth who have better vision than 20/70, and no other concerning vision-related diagnoses when assessed by an ophthalmologist. The clinic does not provide basic vision assessments for glasses, which an optometrist can provide.
We may redirect or close referrals that do not meet these criteria. If a child or youth has not seen an ophthalmologist, you can refer them to the BC Children’s Hospital Ophthalmology Department as well as the Visual Impairment Clinic, to ensure that they will be seen.

Complete a form
Please complete the referral form (PDF) in full and ensure that all patient information is current. Include consults, reports, and/or chart notes (see referral form for requirements).

If you have not already done so, please let your patient know that we will contact them with an appointment time.
