Cleft Palate & Craniofacial Referral

The Cleft Palate & Craniofacial Program includes specialists from across health care including nursing, pediatrics, oral and plastic surgery, orthodontics, genetics, otolaryngology, audiology, speech and language pathology, and social work.

Please note: A referral from a medical professional is needed to visit this clinic.

1Send referral information

Download and complete the fillable PDF referral form
Fax the referral form and all pertinent information to 604-875-2743.
The referral form can also be emailed to: cpalate@cw.bc.ca

2Include tests and reports

Please include any previous relevant tests, reports or consultations with the referral submission.
Please do not fax clinical photographs as they will not come through clearly on our fax machine.

3Wait for an appointment

Once the referral is received, the clinic will call the patient to book the appointment.
All referrals are triaged.
Please note, the current wait time for non-urgent appointments can be from weeks to months.
