To refer a patient to the Neurology Clinic, please follow the referral process and use the referral form below.

Have your child's doctor fill out the referral form and fax it to 604-875-2285

Please include any previous relevant tests, reports and consultations with the referral.

Wait for the neurology physician to triage the referral request.

Once the referral is received and triaged, the clinic with contact you and your family to book an appointment.
