Please write referral letter, making sure to attach all relevant patient data.
2Fax letter to us.
Fax the form to our MOT office at 604-875-2943. We will respond within a few days to say whether the referral has been accepted, is pending receipt of more information, or has been declined.
3Let your patient know that we will be in contact with them.
If you have not already done so, let your patient know that we will contact them with an appointment time.
